Can you help us test the new NHS App?
We are one of the first practices in the country to test the new NHS App. We’d like you to try it and tell us what you think. The NHS App provides a simple and secure way to access a range of healthcare services on your smartphone or tablet. Once registered, you will be able to book and manage appointments at our practice, order your repeat prescriptions, securely view your GP medical record, and much more. Your feedback is very important. It will help the NHS to improve the app before we launch it to patients across the country. If you use Patient Access you can continue to use it. Log into Patient Access Log into Evergreen (previously I-patient)
GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:
- book or cancel appointments online
- renew or order repeat prescriptions online
- view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results
You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. You currently have a choice of 2 providers for access to your medical records online, these are Patient Access and Evergreen (previously I-Patient)
How to sign-up
For more information, please email Patient Services contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up. For more information, please go to the New Patients Section.
Registration Document Emails. A copy of your registration document will be emailed to you once your application has been processes. It will be addressed from @myhealthcareprovider.com. If you have not received this within 7 days of your request, please check your junk / spam folder as it may be there. If it it not there, please let us know and we can provide the details in an alternative format.
Guide to Registering for Patient On-Line Services (patient access specific)
Please Note: When ordering a repeat prescriptions online you should be taken to a screen to confirm your request once you have submitted it. If you do not see this confirmation screen, please contact the surgery to see if we have received it. You can also view your past sent requests from the 'Home Menu', under 'Prescription', 'View Request'.
If you are having problems logging into your account
Both sites have frequently asked questions in their help sections. You can access their help section by clicking the link near the log in box.
If you have forgotten your password, both sites have a forgotten password function. Click the link next to the log in box and you will be asked to verify security questions that you set up during your registration process. This will then allow you to create a new password (the practice doesn't have any access to these security questions so if you have forgotten them we will need to reset your account).
If the of the above doesn't fix your problem we will need to reset your account. please contact the practice if this is the case. We will issue you with a new registration document.
If you are using Patient Access you will need to go through the registration process again. (please see the document above 'Guide to registering for patient on-line Services (page 6 onwards) for more details on how to do this)
If you are using Evergreen you will keep the same log in details but will need to unlink and then re-link your account (information on doing this is given in the document linked below)
